IV Therapy Services Terms and Conditions
Please review the applicable MyCare service terms and conditions before booking, purchasing, or receiving any service. These terms explain eligibility requirements, medical assessment, cancellations, refunds, privacy, safety responsibilities, and service limitations.

IV Therapy Services ( Terms and Conditions)
By signing this document, the patient acknowledges that they have read, understood, and agreed to the following Terms and Conditions relating to Intravenous (IV) Therapy services provided by the facility.
1. Eligibility for Treatment
1.1. All IV Therapy services are subject to medical assessment and approval by a licensed healthcare professional.
1.2. The facility reserves the right to decline, postpone, or discontinue treatment if it is deemed medically inappropriate, unsafe, or contraindicated.
1.3. Patients must provide complete, accurate, and truthful medical information, including medical history, allergies, medications, supplements, and existing health conditions.
2. Informed Consent
2.1. The patient acknowledges that IV Therapy is a wellness and supportive treatment and is not intended to diagnose, treat, cure, or prevent any disease unless specifically prescribed by a licensed physician.
2.2. The patient understands the nature, benefits, limitations, and potential risks associated with IV Therapy and has had the opportunity to ask questions before treatment.
2.3. The patient provides voluntary consent to receive the recommended IV Therapy treatment.
3. Risks and Possible Side Effects
The patient understands that although IV Therapy is generally safe when administered by qualified healthcare professionals, potential risks and side effects may include but are not limited to:
- Pain, bruising, redness, swelling, or irritation at the injection site
- Bleeding or hematoma formation on injection site
- Infection
- Any allergic or hypersensitivity reactions
- Dizziness, nausea, headache, or lightheadedness
- Phlebitis (inflammation of the vein)
- Extravasation or infiltration of IV fluids
- Changes in blood pressure
- Rare but serious adverse reactions
The patient agrees to immediately notify the healthcare provider of any discomfort or adverse symptoms during or 24-48 hours after treatment.
4. Medical Disclaimer
4.1. Individual results may vary significantly and no guarantees are made regarding the effectiveness or outcomes of any IV Therapy treatment.
4.2. The facility does not guarantee specific health, wellness, performance, weight loss, anti-aging, immunity, hydration, or recovery outcomes.
4.3. IV Therapy is a supplementary treatment only and should not replace routine medical care, prescribed medications, or treatment recommended by a physician.
5. Patient Responsibilities
The patient agrees to:
- Follow all pre-treatment and post-treatment instructions.
- Inform the healthcare provider of any changes in medical condition.
- Disclose pregnancy, breastfeeding status, recent surgeries, chronic illnesses, or new medications.
- Notify the facility immediately if any adverse reactions occur after treatment.
6. Appointments and Cancellations
6.1. Appointments may be rescheduled or canceled with at least 24 hours notice.
6.2 No refunds shall be made for cancellations.
6.3. Rescheduling of IV Therapy services shall be deemed valid only for fifteen (15) days due to the “beyond-use-date” of the compounded IV medications. Once the fifteen days have passed, the medication is not safe to use and shall be discarded accordingly.
6.4. The facility reserves the right to reschedule appointments due to operational, clinical, or safety reasons.
7. Mycare Home Healthcare Services
7.1. As it is a home healthcare service, the patient shall provide a safe, clean, and accessible environment for treatment.
7.2. The company and its healthcare professionals reserves the right to refuse service, or may discontinue or service if the environment poses a safety risk.
8. Payment and Refund Policy
8.1. All treatments and packages are offered on a prepaid basis and must be settled in full before the commencement of services.
8.2. Appointment shall be confirmed once the payment has been settled.
8.3. IV Therapy services that have been administered are non-refundable.
8.4. IV therapy services are non-transferable and non-refundable as it is a medical service specific to the client that was assessed and consulted with the physician.
9. Confidentiality and Privacy
9.1. Patient information will be handled in accordance with DHA privacy regulations and facility policies.
9.2. Medical records may be maintained as required by applicable laws and regulatory authorities.
10. Emergency Situations
10.1. In the event of an adverse reaction or medical emergency, the healthcare team may initiate appropriate emergency measures and arrange transfer to a hospital if necessary.
10.2. Any costs associated with emergency transport, hospitalization, or specialist care shall be the responsibility of the patient.
11. Limitation of Liability
To the fullest extent permitted by applicable law, the facility, its physicians, nurses, and employees shall not be liable for any indirect, incidental, or consequential damages arising from the patient's decision to undergo IV Therapy, except where caused by negligence, misconduct, or violations of applicable healthcare regulations.
12. Governing Regulations
All IV Therapy services shall be provided in accordance with applicable healthcare regulations, licensing requirements, and professional standards of practice in the jurisdiction where the services are delivered.
13. Acknowledgment
By signing below, the patient confirms that:
- They have read and understood these Terms and Conditions.
- They have disclosed all relevant medical information concisely and truthfully.
- They have had the opportunity to ask questions and receive satisfactory answers.
- They voluntarily agree to receive IV Therapy services and accept the associated risks.
