Please review the applicable MyCare service terms and conditions before booking, purchasing, or receiving any service. These terms explain eligibility requirements, medical assessment, cancellations, refunds, privacy, safety responsibilities, and service limitations.

Peptide Therapy (Terms & Conditions)
By booking, purchasing, or receiving Peptide Therapy services from Mycare Home Healthcare, the Client acknowledges and agrees to the following Terms & Conditions.
1. Medical Assessment and Eligibility
1.1. All Peptide Therapy services are subject to a medical consultation, assessment, and approval by a licensed physician. This can be done through a phone consultation.
1.2. Treatment eligibility is determined based on the Client's medical history, current health status, laboratory results (if applicable and required), and clinical suitability.
1.3. The Facility reserves the right to decline, postpone, modify, or discontinue treatment if deemed medically inappropriate or unsafe.
2. Prescription Requirement
2.1. Peptide Therapy is provided only upon prescription and clinical approval by a licensed physician where required by applicable regulations.
2.2. Treatment plans, dosages, and duration may be adjusted based on clinical response, laboratory findings, and physician recommendations.
3. Treatment Disclaimer
3.1. Peptide Therapy is individualized. Results may vary significantly between patients.
3.2. Mycare Home Healthcare does not guarantee specific outcomes, including but not limited to weight loss, muscle gain, anti-aging effects, improved energy, enhanced recovery, sleep improvement, or other health benefits.
3.3. Peptide Therapy is a supplementary wellness product. It is not a substitute for routine medical care, prescribed medications, a balanced diet, exercise, or other physician-recommended treatments.
4. Client Responsibilities
The Client agrees to:
- Provide complete, concise and accurate medical information.
- Disclose all current medications, supplements, allergies, and medical conditions.
- Inform the healthcare provider of any changes in health status during treatment.
- Follow all prescribed treatment instructions and monitoring requirements.
- Attend scheduled follow-up appointments and laboratory assessments when required.
5. Peptide Product Ordering, Delivery, and Storage
5.1. Certain peptide products may be specially ordered, compounded, prepared, or allocated specifically for the Client, including customized peptide combinations, peptide pens, or stacked protocols prescribed by the treating physician.
5.2. Peptide products are compounded and supplied through MyCare's partner compounding pharmacy. Upon receipt of full payment, the ordering process will commence. Depending on the peptide product prescribed, delivery may take approximately twenty-four (24)to forty-eight(48) hours, subject to product availability and pharmacy processing times.
5.3. Due to product-specific storage requirements, stability periods, and expiry limitations, peptide products may require refrigeration and handling in accordance with the instructions provided by the Facility. The Client shall receive storage and administration instructions upon delivery.
5.4. The Facility shall not be responsible for product loss, spoilage, contamination, reduced effectiveness, or damage resulting from improper storage, handling, transportation, or use by the Client after delivery.
6. Order Cancellations and Refunds
6.1. Once payment has been received and the peptide product ordering process has commenced, the order cannot be cancelled.
6.2. Peptide products that have been ordered, compounded, prepared, allocated, dispensed, or delivered specifically for the Client are non-refundable and non-transferable.
6.3. If the client wishes to cancel the product after it has been ordered, the client shall remain responsible for the full cost of the peptide product.
6.4. Peptide products procured for a specific Client cannot be returned, reused, reassigned, or resold due to regulatory requirements, quality assurance standards, and product-specific storage and handling conditions.
7. Payments and Refunds
7.1. As peptides are specifically compounded, all peptide therapy services are provided on a prepaid basis.
7.2. Peptide products, Consultation fees, laboratory testing fees, and services already rendered are non-refundable.
7.3. Peptides that are prescribed to the specific client is non-transferable as it is a medical service specific to that client that was assessed and consulted with the physician.
7.4. Refund requests, where applicable, shall be reviewed on a case-by-case basis and subject to Facility approval. Valid reasons for refund requests shall only be considered if there is fault in prescription, dosaging and medicine dispensed.
8. Adverse Reactions and Medical Emergencies
9.1. Clients must promptly report any unexpected symptoms, side effects, or adverse reactions to the referring healthcare professional.
9.2. In the event of a medical emergency, the client shall contact emergency care immediately.
9.3. Costs associated with emergency transport, hospitalization, specialist consultations, or additional treatment shall be the responsibility of the Client.
10. Privacy and Confidentiality
10.1. All personal and medical information shall be handled in accordance with applicable healthcare privacy laws and Facility policies.
10.2. Medical records may be retained and disclosed where required by law, regulatory authorities, or clinical necessity.
11. Limitation of Liability
To the fullest extent permitted by applicable law, the Facility, its physicians, nurses, employees, and representatives shall not be liable for outcomes arising from undisclosed medical conditions, failure to follow medical instructions, improper medication storage, non-compliance with prescribed treatment plans, or circumstances beyond the Facility's reasonable control.
12. Regulatory Compliance
All Peptide Therapy services are provided in accordance with DHA healthcare regulations, professional standards, licensing requirements, and prescribing guidelines.
13. Acceptance of Terms
By booking, purchasing, or receiving Peptide Therapy services from Mycare Home Healthcare, the Client confirms that they have read, understood, and agreed to these Terms & Conditions.
Important Notice: Peptide Therapy requires a separate informed consent form. Acceptance of these Terms & Conditions does not replace the requirement for physician evaluation, prescription (where applicable), or informed consent prior to treatment.
