GLP-1 Weight Loss Program Terms & Conditions
Please review the applicable MyCare service terms and conditions before booking, purchasing, or receiving any service. These terms explain eligibility requirements, medical assessment, cancellations, refunds, privacy, safety responsibilities, and service limitations.

GLP-1 Weight Loss Program Terms & Conditions
By enrolling in, purchasing, or participating in the GLP-1 Weight Loss Program offered by Mycare Home Healthcare, the Client acknowledges and agrees to the following Terms & Conditions.
1. Program Eligibility and Medical Assessment
1.1. Participation in the Program is subject to a medical consultation, health assessment, and approval by a licensed physician.
1.2. A Free Eligibility call shall be scheduled and client inquiries shall be directed to the clinical care team. Eligibility may depend on the Client's medical history, current medications, laboratory results, body mass index (BMI), and overall clinical suitability.
1.3. The Facility reserves the right to decline, postpone, modify, or discontinue participation if deemed medically inappropriate or unsafe.
2. Prescription and Treatment Plan
2.1. GLP-1 medications are prescription medications and will only be provided following physician assessment and approval, in accordance with applicable regulations.
2.2. The Program may include consultations, laboratory testing, medication management, injections, nutritional guidance, and follow-up appointments as determined by the treating physician.
2.3. Treatment plans, dosages, and medication schedules may be adjusted based on clinical response and safety considerations.
3. Program Disclaimer
3.1. The Program is intended to support medically supervised weight management and healthy lifestyle changes.
3.2. Weight loss results vary significantly between individuals, and no specific outcomes or timelines are guaranteed.
3.3. The Program does not replace routine medical care, emergency treatment, or other physician-recommended therapies.
4. Client Responsibilities
4.1. The Client shall provide complete, concise and accurate medical information.
4.2. The Client shall truthfully disclose all medications, supplements, allergies, and health conditions.
4.3. The Client shall follow prescribed dosing, nutrition, activity, and follow-up instructions.
4.4. The Client shall attend scheduled consultations and follow up consultations when required.
4.5. The Client shall promptly report side effects, pregnancy, hospitalization, or significant health changes.
5. Medication Ordering, Delivery, and Storage
5.1. GLP-1 medications may be specially ordered, allocated, or prepared specifically for the Client following physician approval and receipt of payment. This shall be done with the company’s official partner pharmacies for dispensing.
5.2. Delivery timelines may vary depending on medication availability, supplier lead times, and pharmacy processing requirements.
5.3. Certain GLP-1 medications require refrigeration and handling according to the instructions provided by the Facility. The Client is responsible for proper storage after delivery. The dispensing provider will inform the client of the storage instructions of these medicines.
5.4. The Facility is not responsible for medication spoilage, loss, contamination, reduced effectiveness, or damage resulting from improper handling or storage by the Client.
6. Appointments, Cancellations, and Refunds
6.1. Appointment cancellations or rescheduling requests should be submitted at least forty-eight (48) hours in advance.
6.2. Once GLP-1 medication has been ordered, allocated, prepared, dispensed, or delivered for the Client, the medication cost is non-refundable and non-transferable.
6.3. Package purchases are non-refundable and non-transferable unless otherwise required by applicable law.
6.4. Consultation fees, laboratory testing fees, and services already rendered are non-refundable.
7. Program Duration and Packages
7.1. Program packages and sessions, if applicable, are valid only for the duration stated at the time of purchase.
7.2. Unused sessions or benefits expire at the end of the stated validity period and are not redeemable for cash.
7.3. The Facility reserves the right to modify Program pricing, package structure, or included services with prior notice.
8. Fees, Payments, and Medication Charges
8.1. All Program fees, including screenings and packages regardless of the program duration chosen, shall be paid in advance and must be settled in full prior to commencement of services unless otherwise agreed in writing by the Facility.
8.2. The Program fee covers only the services specifically included in the selected Program package, which may include consultations, assessments, follow-up appointments, monitoring, and other designated services.
8.3. GLP-1 medications are charged separately from the Program fee unless expressly stated otherwise in the selected package.
8.4. Medication pricing is subject to prevailing supplier, pharmacy, and DHA regulated prices and may change from time to time. The Facility will communicate the applicable medication cost to the Client prior to procurement.
8.5. Upon receipt of payment for the prescribed medication, the Facility shall initiate the ordering, allocation, procurement and delivery process on behalf of the Client.
8.6. Any laboratory investigations, diagnostic tests, delivery charges, or additional services not expressly included in the Program package may be charged separately.
9. Home Healthcare Services
9.1. For any home-based visits, the Client shall provide a safe, clean, and accessible environment for treatment.
9.2. The Facility may postpone or discontinue services if safety concerns arise.
9.3. Additional travel or service, delivery fees may apply depending on location and scheduling requirements.
10. Adverse Reactions and Medical Emergencies
10.1. GLP-1 medications may cause side effects, including but not limited to nausea, vomiting, diarrhea, constipation, abdominal discomfort, dizziness, injection-site reactions, and other known or unknown adverse effects.
10.2. For non-emergency, non-life threatening symptoms, the client must inform the facility and must be redirected to the assigned physician.
10.3. In the event of a medical emergency, the client shall directly contact the emergency services.
10.4. Costs related to emergency transport, hospitalization, specialist consultations, or additional treatment remain the responsibility of the Client or their insurer.
11. Privacy and Confidentiality
11.1. Personal and medical information will be handled in accordance with DHA healthcare privacy laws and Facility policies.
11.2. Medical records may be retained and disclosed where required by law, regulatory authorities, or clinical necessity.
12. Limitation of Liability
12.1. To the fullest extent permitted by applicable law, the Facility, its physicians, nurses, employees, and representatives shall not be liable for outcomes arising from undisclosed medical conditions, failure to follow medical instructions, non-compliance with treatment plans, improper medication storage, or circumstances beyond the Facility's reasonable control.
12.2. The Client understands that participation in the Program is voluntary and assumes the inherent risks associated with medical weight-loss treatment and injectable medications.
13. Regulatory Compliance
13.1. All services and medications provided under the Program are subject to applicable healthcare regulations, prescribing requirements, and professional standards.
13.2. The Facility reserves the right to suspend or modify the Program to remain compliant with regulatory guidance and medication availability.
14. Acceptance of Terms
14.1. By enrolling in, purchasing, or participating in the GLP-1 Weight Loss Program, the Client confirms that they have read, understood, and agreed to these Terms & Conditions.
14.2. The Client further acknowledges that a separate informed consent form and medical assessment are required prior to treatment initiation.
